Default Bugcode_usb_driver

Hello,
My USB device have a problem with vista sp1 32bit.After turning off my
device, the corresponding items in the device manager fail to
refresh(even though I re-attach my device to the other usb ports of the
motherboard),and the system crashes in a few seconds. The system says
"BUGCODE_USB_DRIVER",and Stop Error is
"***STOP:0*000000FE(0*00000008,0*00000006,0*000000 06,0*xxxxxxxx) ".The
phenomenon could be reproducted each time.By the way ,my device works
fine with XP SP2.
About My Device:
It is a usb device and comprised of a hub with four sub-devices
attached. You could see four sub-device items and a hub item in the
device manager after driver has been installed.

I also have tried another three tests as follows:
Test One:
I follow the same steps as forwards in the safe model of vista, but the
system works fine and the device manager freshes normally while I turn
on and off my device .
Test Two :
If I attach my device to computer through another hub(Not directly
attached to computer),then the device manager freshes correctly when I
power on and off my device. And the system is OK.
Test Three:
If driver isn't installed to my devcice .the device menager freshed
correctly everytime.
Could you tell me what's the matter? what is the main cause?bug of OS
,bug of my device driver ,or my Hardware device doesn't work fine with
vista os ? you could tell me how to locate the problem and solve it as
well.
Your any help will be appreciated.
by Kevin.
