Default hanging process

Everything has slowed down! Most noticeably if I launch qw.exe, close it,
and then attempt to re-launch I cannot do it(error saying already running).
Task manager shows the qw.exe process is still running and gobbling up
memory! Killing process fixes it until the next time. Firefox has also been
behaving similarly.

Tried safe mode and same thing happens.

QW is Deluxe 09 R6 and Vista is Home Premium SP2 with all of the updates
installed.

How do I troubleshoot this further? Virus?(I have Mcafee)

Start with the basics:

The First Question Of Troubleshooting: If the problem is new, what changed
between the time things worked and the time they didn't?

The Second Question of Windows Troubleshooting: what is the malware/virus
status of the machine? If you think it is clean, what programs (and
versions) did you use to determine this?

Be sure the computer is clean:
