Default duplicate users are shown on log in

whenever i turn on the computer, and the log in window appears, my username
appears twice. How do i get this to show only one? I have tried going to
control panel and only one user is shown. Then when i tried to see if was
only my account that did that, i turned on guest account, and i saw two guest
accounts on the log in page. How do i get rid of the duplicate log in names?

Note: I recently upgraded from Windows Vista Business to Ultimate
