My fax modem doesn't show up automatically in my list of printers when I go
into the Print dialogue.

How do I add it?

I tried the normal "add printer" but I was mystified when I had to choose a
port and then never saw any options for fax modems.

What version of Vista do you have? Fax and Scan is only included with Vista
Business and Ultimate. If you have any version of Vista Home, you will need
to get a third party Fax application.
